Probation & Appeals Forms

Due 9-11-20 (to be placed on probation for fall 2020) and 1-8-21 (to be placed on probation for spring 2021).

Withdrawals & Resignations Forms
Note: A withdrawal is initiated by the Campus Director, usually for GPA or hours violations, and a resignation is initiated by the Teaching Fellow.

Please submit withdrawals as soon as possible. Early notification of withdrawals may allow us to replace a freshman or first semester sophomore and/or will eliminate excess paperwork related to the transfer and return of fellowship funds. Due 8-14-20 (withdrawals from summer 2020) and 1-8-21 (withdrawals from fall 2020) and 5-14-21 (withdrawals from spring 2021).

Submit at any time - requests will be presented at the next Board of Director's meeting (Requests must be approved by Jenna Hallman before submission for Board review).

Summer School Funding - Must be submitted by 6/01/2021 for a student to use funds not requested in fall 2020 or spring 2021 for summer 2021.
|
The Fifth Year Grad School form should be submitted when a Teaching Fellow who majored in content over the course of four years, as required by his/her degree program at his/her institution, will need to complete a fifth year program in order to be eligible for teacher certification. Teaching Fellows enrolled in fifth year programs are not eligible for additional funding. This form should be submitted at the end of the Teaching Fellow's senior year.

The Grad School/Early Grads form should be submitted when a Teaching Fellow who majored in content, as required by his/her degree program at his/her institution, completed the undergraduate program in three or fewer years and will begin a required Master's level program in order to receive teacher certification. These Teaching Fellows may be eligible for additional funding if they remain at their TFI and if they have not yet used the full amount of fellowship offered when they accepted the Teaching Fellows award. Teaching Fellows who receive additional funding must continue to attend Teaching Fellows meetings and events. This form should be submitted at the end of the Teaching Fellow's undergraduate program.

The Anticipated Graduation form should be submitted whenever a Teaching Fellow will require additional time to complete his/her undergraduate program. These Teaching Fellows are not eligible for additional funds and are not required to continue attending Teaching Fellows meetings and events.

Teaching Fellows must transfer before the end of their first semester of their sophomore year.
